Targeted Case Management

Case Management Services are designed to assist recipients in gaining access to needed resources and services (medical, social, educational, vocational, financial, and other necessary services) as they relate to the recipient’s mental health needs.  Staff will meet with you and assist in identifying resources and services that you would benefit from such as benefit assistance and medication management.

Who Is Eligible?

You must be 18 years or older, a resident of Kittson, Mahnomen, Marshall, Norman, Polk or Red Lake County and have experienced serious or persistent mental illness.

How Much Do These Services Cost?

Community Support Services are free to eligible people. The program is funded through grants, as well as state and county Medical Assistance.

How Do I Get These Services?

Referrals are accepted from yourself, family, friends, and service providers. To make a referral or get more information, call your county Social Service Center or:
